Every player must submit BOTH order forms. Everyone must fill out the catalogue order form for a dryfit shirt because those and included in your fees, as is the entire uniform order. Please note that everyone is encouraged to come and see the jersey sizes and get fit for a jersey. You should bring shoulder pads, etc to try under jersey so you can ensure fit. Jerseys are reversible and customized when they arrive. You cannot reorder your jersey if you choose the wrong size
